A 70-year-old woman died in November following a ride on the Revenge of the Mummy indoor roller coaster at Universal Studios in Orlando, Florida.

News of the unidentified woman’s death was detailed in a Florida Department of Agriculture and Consumer Services quarterly report on theme park injuries, the Orlando Sentinel reported on Jan. 16.

The woman reportedly became unresponsive while riding the indoor roller coaster on Nov. 25 and later died at a nearby hospital. The Florida government report “reflected only the information reported at the time of the incident,” and the agency “does not receive updates to initial assessments of a patron’s condition.”

Universal Orlando media representatives did not immediately respond to USA TODAY questions about the incident.

A Florida NBC affiliate, WFLA, reported 21 incidents have been linked to the Revenge of the Mummy ride since its 2004 opening, including reports of nausea, dizziness and a vertebrae fracture.

Universal Studios Florida’s website describes Revenge of the Mummy, located in the New York area of the theme park, as a “plunge high speed into total darkness.”
